Lasers and light sources deliver a calibrated light energy, absorbed selectively by certain targets in the skin: the water contained in the tissues, the pigments or the superficial vessels. This absorption produces a tissue reaction which aims to call upon the natural mechanisms of skin repair. Depending on the technique chosen, the action may be ablative, when the surface of the skin is itself treated, as with the CO2 laser, or non-ablative, the epidermis then being preserved. I give preference to the procedure whose indication is established and which matches the characteristics specific to each person’s skin, since one and the same appearance may arise from different mechanisms.

Convinced that a well-conducted procedure begins with a carefully set framework, I begin the session with a cleansing of the area and the fitting of eye protection. Depending on the technique and the region treated, the prior application of an anaesthetic cream may be offered. Passes are then carried out over the area, accompanied, where this is useful, by cooling of the skin. Sensations vary from one person to another, often described as a heat, a tingling or a warming. The duration of the session, the parameters used and the possible pacing of the sessions are a matter for the appraisal made during the consultation, tailored and case by case.

These treatments may concern a dull complexion, irregularities of texture or diffuse redness, as part of a photorejuvenation approach. They are suited neither to every skin nor to every time of year, and the phototype is assessed beforehand. Certain medical histories, the taking of photosensitising medicines or recent sun exposure may lead to postponing the procedure, or even to setting it aside, since my objective is to offer a procedure that is safe, measured and suited to each situation. The areas concerned are defined with each patient.

After-effects may occur. Redness, a sensation of heat, swelling, tenderness or peeling are possible; their intensity and their duration depend on the technique used and on the reaction specific to each skin, and cannot be announced in advance. Sun avoidance and photoprotection are necessary, and the care to be applied is explained to each person. No result can be promised, as the way things develop varies according to the situation. A prior consultation is mandatory: it makes it possible to examine the skin, to take each patient’s medical history, to inform them of the alternatives and the risks, and to check that the procedure is indicated, since it may not be.
